An initial production item is useful only when the review has a defined purpose. Without one, a first-off sign-off can become a general expression of confidence rather than a recorded decision.
ASQ describes quality plans as specifying processes, resources and related responsibilities for meeting requirements. ASQ — quality plans.
Select what the review addresses
Identify the characteristics and evidence to be considered at this stage. Refer to the current approved requirements and relevant sample. Avoid implying that one appearance review establishes every product or process requirement.

Make the disposition explicit
Record what was checked, the observed result and any unresolved condition. If a change is required, identify which evidence must be revisited. The review should show whether production can proceed under the applicable plan, not merely that somebody saw an item.
This gives later investigations a usable baseline. It also prevents an initial review from being cited outside the characteristics and conditions it actually covered.
